Access to medical records
The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.
Complaints policy
Complaints most frequently arise from a misunderstanding or lack of communication. We are happy to discuss any complaint that you may have, and would also be happy to consider any constructive suggestions for improvement of our services. Our aim is to give the highest possible standard of service and we try to deal swiftly with any problems that may occur.
We follow the NHS Lothian guidelines for dealing with complaints. Leaflets are available from our reception or via this site.
If you would like to make a complaint regarding the surgery or the services we offer, please contact the Practice Manger by telephone or, if you prefer, in writing. Every effort will be made to answer your concerns within the agreed timescale.

Health Management
ANNUAL HEALTH CHECKS
The practice now offers flexibility when it comes to your annual health checks. We recognise that many patients now have their own blood pressure monitor and prefer to submit a reading as opposed to attending the surgery.
If you prefer this option please select the form below that was indicated in your reminder text / letter. Once completed you can either hand the form back to the surgery or email it to Loth.HealthInformation78166@nhslothian.scot.nhs.uk The forms are checked and we will contact you if any follow up is required.
If you prefer to see the Health Care Assistant or Practice Nurse for your check we are more than happy to see you as previous. Please contact our reception desk on 01506 670027 to arrange a suitable appointment.
IN HEALTH BP MONITORING (As directed by the practice only)
You recently submitted a blood pressure reading which was above desirable range. It has been advised that you monitor your blood pressure using the ‘In Health Monitoring Service’. In Health is a free communication system that uses text messages to help you and your GP/Nurse monitor your blood pressure more closely.

Initially, you will be asked to check your blood pressure twice a day for 2 weeks. Once this has been completed you will then be contacted by a member of the Practice to advise you of next steps.
